Holiday Distribution |
Beginning in 2009, anyone requesting assistance must submit an application. Please print out the application and mail back to: Hearts of Gold P.O. Box 7806 Algonquin, IL 60102. The application will be reviewed and you will be called for an interview. Upon acceptance you will be issued a client identification card. This card and a photo id will be required when receiving any assistance from Hearts of Gold. * All clients must re-qualify each calendar year.
